The developers describe the content as follows:

As a horror game, Worse Than Death contains scenes of violence, including low-resolution pixel graphics in the environment, bodies (low- and high-resolution), blood-stained walls, flowing blood, and sounds suggestive of violence (creaking, screaming, etc.). In one scene, the player discovers another character hanging by a rope around their neck, swinging back and forth. The player never witnesses an explicit act of violence (e.g., no one dies; only the aftermath is shown).

Although the game has received an ESRB rating for “sexual themes,” these are limited to: allusions to a relationship between two characters; and a character’s confession of possible love for another.
